Grades K-5 and ALC

Online Directions:  You may purchase insurance for your K-5 and/or ALC student online for $5 per device. You may need the serial number from the device to purchase. Purchase the insurance online here

Deadline for payment online is April 10, 2020

Pay in person or by mail: You may pay by cash or by check by completing the form below and sending the money in by mail to the school district office or by dropping off at Halverson.  

Drop-off date deadline is April 6, 2020

Select Foods EmployeeIf the parent or guardian of  a student is a Select Foods employee, please complete the Select Foods Employee form below. Select Foods will be paying the cost of the insurance for their employees who complete this form. Either drop it off at your student’s school or email a copy to your building principal or teacher.
